You will be requested to input the following information:

  • nominator information (your information)
    name, e-mail address, contact information, etc.
  • candidate information
    name, contact information, additional documents (up to 10 files: papers, cv, publication list, etc.)
  • principal reasons for nomination (max: 30 words and 255 characters)
  • detailed description of reasons for nomination (fill a form or upload a file)
